What is it?
This form terminates access to UT Health Science Center systems for employees who are leaving in poor standing, e.g., being terminated for cause. If the answer is yes, submitting the Expedited NetID Disablement form is appropriate.
If the employee is leaving in good standing or has passed away, the Expedited NetID Disablement form is inappropriate. If possible, we encourage the department to work out the details with the employee who is leaving. Recommendations include setting up an auto-reply with new contact information; removing permissions for any departmental resources; transferring ownership of any personal devices, transferring ownership of any Organizational NetID’s, etc. A dean, director, or department head (DDDH) may submit a memo to ITS requesting access to a former employee's account. This can be done without disabling the account. The user does need to be in the loop if we're not revoking access. There might be FERPA implications or other data implications on the account if it continues to be in use - so the user needs to know if someone else has access. (Student info, protected info, etc.)
